	The Ryan Shuman Foundation presents The 2008 Ryan Shuman Strength, Courage, and Determination Award and Scholarship Coaches: Below, you will find an application for the second annual STRENGTH, COURAGE, and DETERMINATION Award and Scholarship. This award is in memory of Ryan J. Shuman, a senior Hockey Player from Eden Prairie that lost his life to a twenty‐nine month battle against a rare form of cancer. Throughout Ryan’s life, especially during his fight against his cancer, he showed a great deal of strength, courage, and determination to meet his challenges each and every day with a positive attitude. Ryan also spent many hours volunteering and was an avid spokesperson against childhood cancer along with maintaining a high commitment to academics. The award and scholarship winner will be a graduating senior that exhibits these attributes (Strength, Courage, Determination, Volunteerism, and Academic achievement. The winner of this award and scholarship will be presented with a $2,000 scholarship and a crystal iceberg award during the Minnesota Hockey Coaches Hall of Fame Awards Ceremony which will be held during the State High School Hockey Tournament. Please submit one graduating senior from your team that has demonstrated the above attributes. The nominated player must have their completed application no later than February 28, 2008. PLAYER ELIGIBILITY To be eligible for this award, the nominated player must be a senior student/athlete participating on a High School hockey team during the 2007‐08 hockey season. PLAYER NOMINATION Coach, please follow these simple steps: 1. Either email the nomination to info@ryanshumanfoundation.com with the information from SECTION 1 on the application OR fax the application with SECTION 1 to 952‐937‐2511. 2. Notify your nominated player and provide them the application so they can begin preparing the requested information. 3. The foundation will email or mail application to the applicant APPLICATION Application forms are available on the Minnesota Hockey Coaches Association website at www.mshsca.org or from the Ryan J. Shuman Foundation at www.ryanshumanfoundation.com Along with the completed application you must submit the following information (incomplete applications will not be considered): 1. Completed application form; 2. Certified high school transcript showing current grade point average and class rank, if possible. The transcript must include and SAT or ACT test score, if available; 3. Three letters of recommendation. One from your current youth hockey coach, one from local association president or board member and one other teacher or community leader. Recommendation letters should be sent directly to the scholarship committee at the address noted above. Download Applicaton Here Selection Scholarship applicants must meet the following academic criteria to be considered for a scholarship award. 1. Minimum GPA of 2.0 on a 4.0 grading system; 2. Applicant must be considered a student in good standing (not on probation or suspension) at the school they attend; 3. Scholarship awards cannot be deferred.
